By submitting this work order, tenant accepts this notice that the leased premises will be entered to resolve filed issue within the next two business days, during normal business hours (8am to 5pm, Monday-Friday). Should tenant prefer not to have this immediate entry made during this time frame, please supply two time slots below, which is at least two businesses days from now in comments section.
